Medical Forms

Annual Health and Medical Record (Parts A and B only - doctor's signature NOT required) is required for each Scout prior to attending any outdoor activity with the Troop. We are required to bring these forms with us on campouts.

For summer camp or trips longer than 72 hours, Part C is also required for SCOUTS AND ADULTS and REQUIRES A DOCTOR'S SIGNATURE. Some families get the Scout physical form completed at the same time their Scout is getting a sports physical ("two birds with one stone").
