Please note a Lion and Tiger membership packet is different from the Cub Scout membership packet. Lion and Tiger Scouts must have a registered Adult Lion/Tiger Partner and the adult's required paperwork with the packet. Adult Lion/Tiger Partners must be present at all scout activities.
LION and T I G E R P A C K E T (Kindergarten and 1st Grade)
Tiger Checklist
A checklist must be completed and be the first page of your membership packet. The checklist will insure that all paperwork is completed and attached.
Youth Membership Application
This application registers both the scout and the adult partner. The radial button that states, "Mark here if you are the Tiger Adult Partner", must be completed. Adult Tiger Partners must also complete and verify with a certificate that they have completed Youth Protection Training.
- Electronic version can only be printed and then completed in writing.
- Parents must sign the form at the appropriate area at the bottom of the form.
- LION: must write "K" and LION at top of application (no option on application yet)
Annual Health and Medical Record for LION and TIGER
- Part A & Part B
- Immunization records must be included - either written on the form OR attach a copy of your scout's immunization records.
- Copy of Insurance Card - Front & Back
Activity Consent Form
Parents must complete for scout to participate in scouting activities. Dated for the current scouting year August (current year) through December (of next year).
Annual Health and Medical Record for ADULT PARTNER
Requires a copy of your insurance card (front & back) if different from your scout's insurance card.
Youth Protection Training Certificate for ADULT PARTNER
Application cannot be completed without this certificate. Please see Pack 531 Membership Information document below for instructions on how to take this online training and print the required certificate.

- Adults must sign the application page in the designated area at the bottom of the main page of the form.

Parent Training - Youth Protection Training Certificate
Pack Leaders and Committee Members are required to watch the Youth Protection Training online training by creating an account at my.scouting.org and click on E-Learning to take the course. This is a required training by the Boy Scouts of America. The training is approximately 30 minutes. Once training is completed, the certificate must be printed and attached to the membership packet. The training certificate is required to be submitted at the time of application.
